SASSA Documents and Affidavits – Download Them Here

 

When applying for SASSA grants (whether you apply in-person at a SASSA office or online), you will be required to complete specific documentation. One of the documents you will have to complete is an affidavit.

An affidavit is a written declaration signed under oath – the signer has sworn that the statement is truthful and has been informed that they may face legal consequences if the entire affidavit or any portion of it is proved to be false.

You can download the SASSA affidavits here or you can collect them at your nearest SASSA branch when applying – after you have completed the affidavit, you will need to have it stamped by a commissioner of oaths or at a SAPS police station.
 

Documents for applying for SASSA grants

AFFIDAVIT FOR AN OLDER PERSON’S GRANT (ANNEXURE I-1)

This form must be completed by those applying for the old age grant, whereby they confirm that their personal details (full name, ID number, age and physical address) are true and correct. The form also allows for applicants to confirm that they do not reside in a state institution, as well as their marital status and any sources of income or assets owned (their own or their spouses). Lastly, the form must be stamped and signed by a commissioner of oaths/ SAPS and must include the applicants thumb print.
 

AFFIDAVIT FOR A DISABILITY GRANT (ANNEXURE I-2)

This form must be completed by those applying for the disability grant, whereby they confirm that their personal details (full name, ID number, age and physical address) are true and correct. The form also allows for applicants to confirm that they do not reside in a state institution, as well as their marital status and any sources of income or assets owned (their own or their spouses). Lastly, the form must be stamped and signed by a commissioner of oaths/ SAPS and must include the applicants thumb print.
 

AFFIDAVIT FOR A CHILD SUPPORT GRANT (ANNEXURE I-4)

This form must be completed by those applying for the child support grant, whereby they confirm that their personal details (full name, ID number, age and physical address) are true and correct, as well as the details of the child (full name, date of birth, ID number and place of birth).
The form also allows for applicants to confirm that they are the primary care giver or biological parent of the child, the child resides with them and they are not receiving payment to care for the child. If applying for more than 1 child, a list of additional children must be added. Applicants must also confirm their marital status and any sources of income (their own or their spouses). Lastly, the form must be stamped and signed by a commissioner of oaths/ SAPS and must include the applicants thumb print.
 

AFFIDAVIT FOR A CARE DEPENDENCY GRANT (ANNEXURE I-5)

This form must be completed by those applying for the care dependency grant, whereby they confirm that their personal details (full name, ID number, age and physical address) are true and correct, as well as the details of the child (full name, date of birth, ID number and place of birth).
The form also allows for applicants to confirm that they are the parent, foster parent or primary care giver of the child, the child resides with them and the child is not in a state funded institution. Applicants must also confirm their marital status and any sources of income (their own or their spouses).
Lastly, the form must be stamped and signed by a commissioner of oaths/ SAPS and must include the applicants thumb print.
 

AFFIDAVIT FOR A GRANT IN AID (ANNEXURE I-7)

This form must be completed by those applying for the grant-in-aid, whereby they confirm that their personal details (full name, ID number, age and physical address) are true and correct. The form also allows for applicants to confirm that they are recipients of either the older persons grant, war veterans grant or disability grant.
Applicants must confirm that they do not reside in a state institution and that they require regular assistance from another person.
Lastly, the form must be stamped and signed by a commissioner of oaths/ SAPS and must include the applicants thumb print.
 

PROOF OF SCHOOL ATTENDANCE (Annexure E)

When applying for the child support grant, applicants are encouraged to submit the proof of school attendance form, for school going children between 7 and 18 years old.
It is not a strict criteria to submit this form, the grant will still be processed without it, however the government strongly encourage parents to submit this document.
Applicants must confirm the childs/ childrens personal details (full name, ID number), as well as the schools name and contact details, the childs results and their attendance.
Lastly, the form must reflect the principals name and signature, as well as the school stamp.
 

CONSENT FORM FOR BANK PAYMENT

SASSA grant recipients who want their grants to be paid into their bank account (instead the cash collection option at selected stores), must complete this form.
Applicants must confirm their personal details (full name and ID number), as well as their physical address, cellphone number and email address are true and correct.
The form must also include the applicants banking details (name of bank, account number, branch code and account type) and must be accompanied by a bank document to prove these details – such as a bank statement or proof of account.
Lastly, the form must be signed by the applicant.
